*Our clinic is located in a large commercial complex housing both office spaces (Nihonbashi Muromachi Mitsui Tower) and a retail center (COREDO Muromachi Terrace).
Please note that the two areas have separate entrances. All restaurants are located in the retail center.
Terms & Conditions
- The vouchers will be provided upon completion of the health screening appointment.
- Individuals using subsidies from their health insurance societies or discounts from their employee benefits agency are not eligible for this voucher.
- Vouchers are valid for three months from the issue date (appointment day).
- Please present the vouchers to the staff upon arrival at the restaurant.
- Dining options, restaurant menus and hours of operation may change without prior notice.
- The redemption of vouchers is strictly subject to availability. (Please contact the restaurant before use.)
- Vouchers are valid for one-time use only. Multiple vouchers can be used in combination for a single transaction.
- Vouchers that have expired or do not have an issue date will not be accepted.
- Vouchers are non-refundable and cannot be reissued or exchanged for cash in part or full.
- We are not responsible if a voucher is lost, stolen, damaged, or destroyed. No replacement will be provided in these circumstances.
- Please note that no cash change will be given when using the vouchers.
- Vouchers cannot be combined with any other vouchers that have not been earned with the Nihonbashi Muromachi Mitsui Tower Midtown Clinic.
