【Purpose】

We have made a significant revision to our previous questionnaires in order to make AI predictive analysis of multiple diseases including diabetes possible in the near future.

The number of questions will be increased to cover the essential factors to predict lifestyle diseases, however,it will be more convenient because your data will be saved due to web questionnaires that makes you be able to skip certain questions from the next year.

（We do not use all questionnaires for the analysis at this point, however, we recommend you to answer all the questions because it could be used for lifestyle disease improvement program in the future. Answering these questions might give you an opportunity to improve your lifestyle as well.）

In consequence of new system, your personal data to predict lifestyle diseases will be reflected on your medical examination report. We commit to provide the best solutions and excellent service based on your continuous medical examination data.

-Background-
The technical advance of AI enables to compile enormous amount of data effectively. This achievement has been applied in medical field as well.
TOSHIBA developed algorithm which enables to predict diabetes by analyzing medical examination data using AI. Questions related to lifestyle diseases, which had not been used effectively, are now categorized and used along with blood test results. As a result, AI analysis came true. We applied algorithm on the data in our clinic and confirmed that it can be used in this clinic effectively. We have presented our joint research with TOSHIBA as the title of “Development of algorithm to predict diabetes using AI analysis of medical examination data” at annual scientific meeting of Japan Society of Ningen Dock in 2019.

[How to answer]

■ A) Present illness

・Definition of "2. During hospital visit (regular medical examination)" and "3. Untreated or discontinued"

Visiting hospital: Monthly medical examination or next appointment
Suspended: A situation in which a doctor has not been able to see a doctor even though more than a month has passed since the appointment / doctor's instruction to see a doctor.

・About Q1-Q20 and Q25

Please select one from options 1-5 and fill it out. If you have more than one disease and their treatment course is different, please choose one according to the following priorities. The order of priority is "1. During treatment (oral administration, inhalation, application, eye drops, injection, CPAP, etc.)"> "2. During hospital visit (regular medical examination)"> "3. Untreated or discontinued"> "4. healing"
<Example> When one is "1. Under treatment with medication" and the other is "4. Healing", select "1. Under treatment with medication".

■ C) Smoking

The harm of heated tobacco started to be known lately. Questions are separated by types of tobacco you smoke in order for you to recheck your own smoking pattern.

■ D) Family history

Select「１」if you have one family member with applicable disease and the age of onset is under 50. Select「２」if the age of onset is over 50. Select「３」if you have 2 or more family members with applicable disease regardless of age. Select「４」if you are not sure.

■ F) Exercise habit and others

This is for specific health guidance or consultation with doctor. Your answer will be used for AI prediction of lifestyle diseases such as diabetes.

■ I) Subjective symptoms

Q11-14 are also applied to the content of stress check. This is for the purpose of raising awareness of your own condition. If applicable, consultation with doctor, industrial physician or family is recommended.

■ K) Diet

Questions regarding your diet. We do not use some of the items at this point, however, we recommend you to answer all the questions because it could be applied for lifestyle disease improvement program in the future. Answering these questions might give you an opportunity to improve your lifestyle as well.
